Liquid Rhinoplasty: What You Need to Know

Understanding the Hidden Risks of Nasal Fillers

Gregory Dibelius  0:00

Hey everyone, today we’re going to talk about one of the more controversial topics in rhinoplasty, and that’s the issue of fillers in the nose, otherwise known as liquid rhinoplasty or non-surgical rhinoplasty.

Listen, this is just my opinion, but I think they have an extremely limited role in the rhinoplasty world, and I’ll tell you why.

Gregory Dibelius  0:21

First, I think as a rule it should never be put in the tip, the columella, the side walls of the ala, or anything like that.

I think that the risk of vascular compromise, which is injury to the blood supply, is way too high in these areas.  The main vessels that feed the nose come in through sort of the cheek area and then feed the nose near the bottom third of the nose.

So when we put fillers in here, it puts a lot of strain on those areas. It can compress blood vessels. And it can actually kill the skin, which is a devastating problem to have.

So, my general rule is in the bottom third of the nose, absolutely no fillers.

Now there is a role for fillers, I believe, in the top half of the nose. So, on the bridge it’s much easier to put the filler in the right spot where it should not interfere with these blood vessels.

So, that’s filling in the radix, which is the top. That’s a typical liquid rhinoplasty approach to making the nose look straight. I think that’s reasonable.

You still run significant risks, but I think it’s much lower than the risk of injecting the bottom third.

And then sometimes you know there’s little irregularities of the dorsum and things like that that may be appropriate as well, but I do think my personal opinion is just that the tip of the nose is a minefield if you’re going to inject it with filler.
